What Is Medicare Part D?
Medicare Part D is the federal prescription drug benefit available to anyone enrolled in Medicare Part A and/or Part B. Part D coverage is provided through private insurance companies approved by Medicare — it is not administered directly by the federal government.
Part D plans vary by carrier, formulary (list of covered drugs), pharmacy network, and cost structure. Choosing the right plan based on your specific medications can save you hundreds of dollars per year.
How Part D Works
Choose a Plan
Compare Part D plans available in your ZIP code based on your specific medications, preferred pharmacy, and monthly premium.
Pay Your Premium
Most Part D plans charge a monthly premium in addition to your Medicare Part B premium. Premiums vary by plan and region.
Use Your Coverage
Fill prescriptions at in-network pharmacies. Your plan's formulary determines which drugs are covered and at what cost tier.
Track Your Costs
Part D has four cost phases: Deductible, Initial Coverage, Coverage Gap (Donut Hole), and Catastrophic Coverage.
The Four Part D Cost Phases
Understanding how costs change throughout the year helps you plan and budget for your medications.
Deductible
You pay 100% of drug costs until you meet your plan's deductible. In 2025, the maximum Part D deductible is $590. Some plans have a $0 deductible for certain drug tiers.
Initial Coverage
After meeting your deductible, you pay your plan's copay or coinsurance for covered drugs. Your plan and Medicare share the remaining cost.
Coverage Gap (Donut Hole)
In 2025, once your total drug costs reach $2,000, you enter the Coverage Gap. Under the Inflation Reduction Act, your out-of-pocket costs are capped at $2,000 per year — the Donut Hole effectively no longer applies as it did in prior years.
Catastrophic Coverage
After reaching the $2,000 out-of-pocket cap, Medicare covers 100% of your covered drug costs for the rest of the plan year.
Part D Enrollment Periods
Initial Enrollment Period (IEP)
A 7-month window centered on your 65th birthday month (3 months before, your birthday month, and 3 months after). Enrolling late without creditable coverage triggers a Late Enrollment Penalty.
Annual Enrollment Period (AEP)
October 15 – December 7 each year. You can join, switch, or drop a Part D plan. Changes take effect January 1 of the following year. This is the most important window to review your plan annually.
Special Enrollment Period (SEP)
Triggered by qualifying events such as moving out of your plan's service area, losing creditable drug coverage, or qualifying for Extra Help (Low Income Subsidy).
Medicare Advantage Open Enrollment (MA OEP)
January 1 – March 31. If you are enrolled in a Medicare Advantage plan with drug coverage, you can switch to a different MA plan or return to Original Medicare and add a standalone Part D plan.
Late Enrollment Penalty
If you do not enroll in Part D when first eligible and go 63 or more consecutive days without creditable prescription drug coverage, you will owe a Late Enrollment Penalty (LEP). The penalty is 1% of the national base beneficiary premium for each full month you were without coverage. This penalty is added to your monthly Part D premium for as long as you have Part D — it does not go away.
Example: If you went 24 months without coverage, your penalty would be approximately 24% of the national base premium added to your plan's premium each month.
Extra Help — Low Income Subsidy (LIS)
Medicare's Extra Help program (also called the Low Income Subsidy) helps people with limited income and resources pay for Part D premiums, deductibles, and copays. In 2025, individuals with income up to 150% of the Federal Poverty Level may qualify. If you qualify for Extra Help, you also get a Special Enrollment Period to join or switch Part D plans at any time.
